Incorporated associations forms and fees
Access incorporated associations forms, including online lodgment, payment methods and fee information.
Some incorporated association forms can be submitted online, and the lodgement fee is included in this process.
There is no need to use the separate payment portal to pay for these online forms.
Forms available to lodge online
There are 3 forms incorporated associations can lodge online:
- Form A9 - Notice of appointment of public officer and Notice of change of address of association. This form is used to notify an appointment/cessation of public officer or a change of official address of the incorporated association.
- Form A12 - T1 (Large) - Annual summary of financial affairs - Tier 1. An incorporated association is a Tier 1 incorporated association if:
- the gross receipts/total revenue of the incorporated association for the incorporated association’s financial year exceeds $250,000, or
- the current assets of the incorporated association for the incorporated association’s financial year exceeds $500,000.
- Form A12 - T2 (Small) - Annual summary of financial affairs - Tier 2. An incorporated association is a Tier 2 incorporated association if:
- the gross receipts/total revenue (income/money collected) of the incorporated association for the incorporated association’s financial year is less than $250,000, and
- the current assets (cash/bank accounts/shares) of the incorporated association for the incorporated association’s financial year is less than $500,000.
Lodging forms other than online
Completed forms, along with a copy of the lodgement fees receipt, can be lodged in one of 3 ways:
- email to [email protected] with the subject line the full name of the incorporated association and its registration number
- post to PO Box 22 Bathurst NSW 2795
- in person at any Service NSW centre.
Lodgement fees
Lodgement fees for forms can be paid:
- by the payment portal, or
- at any Service NSW centre.
Paying by cheque
We do not accept payments by cheque for forms received by mail.
Some Service NSW centres will accept cheque payment. Call 13 77 88 before visiting to confirm payment options at your chosen centre.

Form A1 - Application for reservation of name
It is not necessary to reserve a name. An incorporated association may reserve a name by lodging Form A1-Application for reservation of name. A name reservation lasts for 3 months.

Form A2 - Application for registration of incorporated association
This form must be lodged in order to incorporate an association where the application is being made on behalf of:
- at least 5 individuals, or an
- existing unincorporated body.
This form must be signed by the person nominated to be the first public officer of the incorporated association.

Form A3 - Application for registration of an amalgamated incorporated association
This form is used where 2 or more incorporated associations wish to amalgamate into one incorporated association. Form A3 must be signed by the nominated first public officer of the amalgamated incorporated association. The public officer of each of the amalgamating incorporated associations must complete a copy of the Supplement to Form A3. Those documents must be lodged with the form.

Form A4 - Application for registration of incorporated association by a registrable corporation
A company, a co-operative or Aboriginal and Torres Strait Islander Corporation making an application for incorporation as an incorporated association under the Associations Incorporation Act 2009 must lodge this form.

Form A5 - Certificate of public officer as to vesting of property
This certificate certifies that property:
- belonged to an unincorporated body, or
- was held by another person in trust for the unincorporated body, immediately before incorporation of the association.

Form A6 - Application to register change of objects or constitution
This certificate certifies that property:
- belonged to an unincorporated body, or
- was held by another person in trust for the unincorporated body, immediately before incorporation of the association.

Form A7 - Application to register change of name
Form A7 must be lodged within 28 days of the passing of the special resolution to change the incorporated association's name. The current certificate of incorporation must be lodged with the Form A7.

Form A8 - Application for voluntary cancellation of registration of an association
This form is used to apply for the cancellation of the registration of an incorporated association. The incorporated association must pass a special resolution approving the application. At the time the application is made the incorporated association must not have any outstanding liabilities.

Form A9 - Notice of appointment of public officer and Notice of change of association address
This form must be lodged:
- by the public officer of the incorporated association within 28 days after taking office as the public officer, or
- within 28 days of a change in the official address of the incorporated association.

Form A10 - Application for approval to transfer registration
This form must be lodged:
- where an incorporated association intends to transfer its registration to a company under the Corporations Act 2001
- by a corporation under the Corporations (Aboriginal and Torres Strait Islander) Act 2006, or
- by a co-operative under the Co-operatives National Law (NSW).

Form A11 - Application for extension of time for holding annual general meeting or lodging annual summary of financial affairs
The incorporated association is required to lodge this form if it is unable to:
- hold its annual general meeting within 6 months after the end of its financial year, or
- lodge the Annual summary of financial affairs by the due date.
Form A12 - T1 (Large) - Annual summary of financial affairs - Tier 1
Tier 1 (large) incorporated associations are required to lodge an annual summary of financial affairs within one month after the holding of the AGM and not later than 7 months after the end of the financial year of the incorporated association, whichever is earlier, unless there has been an approval of an extension of time for:
- holding an AGM, or
- to lodge an annual summary has been approved.

Form A12 - T2 (Small) - Annual summary of financial affairs - Tier 2
Tier 2 (small) incorporated associations are required to lodge an annual summary of financial affairs within one month after the holding of the AGM and not later than 7 months after the end of the financial year of the incorporated association, whichever is earlier, unless there has been an approval of an extension of time for:
- holding an AGM, or
- to lodge an annual summary has been approved.
